Notfallmedizinischer Fortbildungskongress RettCon (11. und 12. Juni)

Thema ist nicht die Indikation zur Beatmung! Sondern: Sofern die Indikation zur Beatmung steht, ist die endotracheale Intubation der Goldstandard - wie gehen wir vor? Und was mache ich, wenn diese nicht gelingt?

Es wird im Vortrag auf die Vorteile der endotrachealen Intubation eingegangen. Wie gelingt uns Aspirationsschutz? Welche Berücksichtigung müssen durch die Nicht-Nüchternheit der Notfallpatienten beachtet werden? Selbst bei Nüchternheit gibt es notfallspezifische Auswirkungen, die uns diesen Patient als nicht-nüchtern betrachten lassen müssen! Zum Beispiel: intraabdomineller Druck bei Notfallpatienten, Schwangerschaft, Verschüttung etc. Weitere Punkte im Vortrag sind das notfallmedizinische Vorgehen als Crash-Intubation, Crash-Induction, Ileuseinleitung, Blitzintubation aber auch Komplikationen bei der Intubation. Besondere intubationserschwerende Umstände in der Rettungsmedizin, Tubusgröße, Tubustiefe, Intubationsbesteck werden kurz thematisiert.

Zentrales Hauptaugenmerk jedoch liegt auf der Frage: Was geschieht beim Misslingen einer geplanten Beatmung? Es folgen zunächst Definitionen der ASA Task Force on Difficult Airway Management und Leitlinien der DGAI zum Atemwegmanagement. Viele eigene und vielfältige Fotodokumente von Beispielen schwieriger Atemwegsnotfällen und tabellarische Übersichten über Atemwegsalternativen generell und speziell im Rettungsdienst folgen. Demonstrationen von Fotos zu Tricks, Kniffen und praktischen Anwenderbeispielen der Tubusalternativen werden gezeigt. Der Super-GAU: "Cannot intubate - cannot ventilate" - so selten er ist, so gefährlich ist er. Mut und Erfahrung lassen uns diese Situation auch in der Präklinik durch verschiedene Methoden an den oberen Atemwegen meistern.

Abschließend werden weitere Atemwegsalternativen gezeigt, welche aus verschiedenen Gründen leider nicht überall Einzug in die präklinische Notfallmedizin finden. Es folgen Merksätze, Zusammenfassung und Trends für die Zukunft.
